Reimbursement Guidelines for non-U.S. Citizens

If you are a US citizen, permanent resident, or resident alien, please see the guidelines for U.S. citizens.

All of the necessary forms can be downloaded together here.

  1. This guideline applies to individuals who are NOT US citizens, permanent residents (with green cards) or resident aliens for US tax purposes.
    You need to determine if you are a resident alien for US tax purposes by yourself. Generally, a nonresident alien means you have been live in the US NO longer than Five calendar years. After five year’s residence, you may become a resident alien and you need to pay tax for all your income.
  2. Reimbursement Forms

    All attendees awarded a financial aid by the SIEYP committee must fill out the following forms and return it with original receipts and airplane boarding passes.

    • Participant Certification
      • For "Stipend Amount", please enter "N/A".
      • For "Reimbursement Amount", please enter the actual amount you requested for reimbursement or the amount granted to you by the SIEYP committee, whichever is less.
      • Please leave what "to be completed by an authorized departmental representative" blank.
    • Alien Data Collection Form
    • Compliance Statement (if you are in B visa status of B1/B2/WB/WT)
    • IRS W-8BEN Form
    • Independent Contractor Expense Reimbursement Form
    • Other Required Documents
      • Photocopy of passport picture page
      • Photocopy of US visa sticker
        • If from a visa waiver country, provide a copy of the US entry stamp for the current visit
      • Photocopy of from and back of I-94 card
        • If you will leave US immediately after SIEYP, MAKE SURE to make a copy for your I-94 before you leave, because I-94 will be collected by the US immigration officials when you depart the US
      • Photocopy of I-20, if you are with F1 visa
      • Photocopy of DS-2019, if you are with J1 visa
        • If you hold a J1 visa, please also obtain and provide us a written permission from your sponsoring institution's International Programs Office in order to receive a payment from Arizona State University
      • All original receipts and airline boarding passes
         
  3. Items can be reimbursed
     
    • For airfare, only economic coach can be reimbursed. For international travelers, please use U.S. air carriers unless airfare of your non-U.S. carrier is cheaper than that of any U.S. carrier (please provide airfare quote proof for reimbursement).
    • For U.S. domestic attendees, only hotel-nights on May 16 (Saturday) and May 17 (Sunday), 2009 can be reimbursed. For international attendees, hotel-nights on May 15 (Friday) through May 18 (Monday), 2009 can be reimbursed.
       
  4. Mailing

    After your return, please collect all original receipts and airplane boarding passes, glue them on a blank paper, and mail back to the following address together with appropriate signed forms by June 12, 2009. Write "SIEYP" on the lower-left corner of the envelope. Please allow three to four weeks for arrival of the reimbursement checks.
